Rankings up, traffic down – what’s really going on?

For most dental practices, rankings, traffic and enquiries move together.

  • If you rank well, traffic follows.
  • If traffic holds steady, enquiries usually do too.

That relationship underpins most SEO reporting.

However, we’ve recently noticed a different pattern across several dental practices. Rankings are holding strong. Enquiries remain healthy. Yet organic traffic has been gradually declining month after month.

To find out whether this was a genuine trend, we reviewed the data across a sample of our clients.

What we looked at

We reviewed 30 dental practices across a single reporting month, comparing three key metrics for each practice:

  • Google rankings – whether they were holding, improving or slipping.
  • Organic traffic – whether it was increasing, decreasing or remaining flat.
  • Enquiries – including contact form submissions, phone call clicks and online bookings.

The logic was simple. If rankings and enquiries remain healthy while traffic declines, it suggests a change in user behaviour. If rankings, traffic and enquiries all decline together, it is more likely to reflect a genuine drop in SEO performance.

What we found

Across the 30 Premier SEO clients we reviewed:

  • 9 practices had steady or improving rankings alongside healthy or increasing enquiry numbers, despite a consistent downward trend in organic traffic.
  • 5 practices experienced declines in both traffic and enquiries, indicating a more typical drop in performance rather than a change in search behaviour.
  • The remaining 16 practices showed stable or improving traffic, with no obvious pattern.

In other words, nearly one third of the practices we reviewed continued to rank well and generate consistent enquiries, even though fewer patients were visiting their website.

One practice illustrates this particularly well. Over the course of a year, organic traffic almost halved while keyword rankings changed very little and monthly enquiries remained close to their usual level. This doesn’t point to a visibility problem.

Why this might be happening

1. AI Overviews and zero click search

If Google AI Overviews, ChatGPT or Gemini answer a user’s question directly within the search experience, a page one ranking no longer guarantees a website visit. Patients may find the information they need, then call or book without ever clicking through to the website.

2. Shorter, more direct search journeys

Someone searching for “emergency dentist near me” may go straight to a Google Business Profile, map listing or phone number instead of visiting the practice website.

This would appear as steady calls and Google Business Profile interactions alongside declining website traffic, which is exactly what we observed for several practices.

3. Fewer but more qualified visits

If AI tools and Google’s search features filter out casual researchers before they click, the people who do reach the website may already be closer to booking an appointment.

That would explain why traffic can fall while enquiry numbers remain consistent.

We cannot prove causation from this dataset alone. However, the overall pattern is compelling. Rankings remain stable, enquiries remain stable, yet raw traffic declines. That points more towards changing search behaviour than a reduction in demand for dental services.

What it doesn’t mean

This isn’t a universal trend.

Five of the 30 practices reviewed experienced declines in both traffic and enquiries, suggesting a genuine drop in performance rather than a shift in how people search.

This isn’t evidence that SEO traffic is disappearing. Instead, it highlights a pattern emerging across a meaningful proportion of practices, while many others continue to perform exactly as expected.

What this means in practice

If your traffic falls but your rankings and enquiries remain stable, it isn’t automatically a cause for concern.

Instead, consider reviewing:

  • Your Google Business Profile performance, including calls, direction requests and website clicks, alongside your website analytics.
  • Whether your most valuable pages, such as treatment pages, fee pages and your contact page, continue to be indexed and receive clicks.
  • Enquiry numbers as your primary measure of SEO success, rather than traffic alone.

Traffic has traditionally been the headline metric in SEO reporting. However, if more users are finding the information they need before visiting a website, it may be time to view traffic as one signal among several, rather than the only measure of success.
