Does it matter if I choose a .com or .co.uk domain?
No, Google has stated that it treats all top level domains (.com, .co.uk etc) the same.
Will it benefit my rankings if I choose a domain with keywords in it?
Google once used keywords in domain names as a ranking factor for websites. It is now less important to have keywords in domain names. What is more important is to have your practice name in the domain name as this is what people are going to recognise in search results.
Can I change my domain name?

I have noticed that .london domain names are now available should I get one?
Google have stated that it treats .london domain names the same as other top level domains (.com, .co.uk etc). However if you want to secure your brand name it is best to purchase your .london domain before somebody else does and you never know Google could change their ranking algorithm at any stage and use the .london extension as a ranking factor for searches carried out in London.
Should I purchase a .dentist/.dental domain name?
If you already have a .com or .co.uk domain name I would say you would want to secure your .dentist/.dental domain for possible future use. I don’t think you should use it for your main website just yet as people aren’t familiar with this domain extension in Google and might be less likely to click through to your website. However, when people become familiar with a whole range of different domain extensions you might chose to use your .dental/.dentist domain as your main domain to try and stand out from the competition.
