The best way for us to create online medical history forms is to use what you already have. No point in reinventing the wheel. If you just send us a version of what you use in the practice, we will customise your online form to use all the fields that you need.
Part of the ‘new normal’ will involve a questionnaire to screen patients before their appointments. While this can easily be a call, there’s no reason why you can’t use an online form to free up your front desk team.

*A new, continuous cough means coughing for longer than an hour, or three or more coughing episodes in 24 hours. If the patient usually has a cough, it may be worse than usual.
