Safe and Savvy Digital Patient Forms for a Paperfree Practice

Even before the pandemic changed the face of dentistry, practices were starting to adopt a fully paperless way of working. The obvious eco-benefit for reducing paper and waste is incentive enough, but now it’s clear that digital communications bring about a safer way to work as well as a green one. 

We can help with swapping paper with digital alternatives, creating a safer and more convenient way for your patients to deliver personal information to you. Online forms are a secure and contact-free alternative to medical history forms and referral forms.

Taking what you have and turning it digital

The best way for us to create online medical history forms is to use what you already have. No point in reinventing the wheel. If you just send us a version of what you use in the practice, we will customise your online form to use all the fields that you need. 

Coronavirus questionnaires

Part of the ‘new normal’ will involve a questionnaire to screen patients before their appointments. While this can easily be a call, there’s no reason why you can’t use an online form to free up your front desk team. 


Have you tested positive for COVID-19 in the last 7 days?

Are you waiting for a COVID-19 test or the results? 

Do you have any of the following symptoms: 

  • New, continuous cough*; 
  • High temperature or fever; 
  • Loss of, or change in, sense of smell or taste? 

Do you live with someone who has either tested positive for COVID-19 or had symptoms of COVID-19 in the last 14 days? 

*A new, continuous cough means coughing for longer than an hour, or three or more coughing episodes in 24 hours. If the patient usually has a cough, it may be worse than usual.
